Dr. Lalezar performs a thorough review of your provided medical history and lab results, ensuring your session is dedicated to personalized, high-value recommendations. Should you wish to include a comprehensive analysis of additional or extensive laboratory data, this enhanced review requires additional physician expertise.

A dedicated 30-minute follow-up consultation with Dr. Lalezar to evaluate your progress and thoughtfully refine your treatment plan. Ongoing follow-up care is included in the Health & Vitality Method.

Important Information
Health & Vitality Center is a root-cause, functional medicine practice that operates outside of insurance.
All services are self-pay and payment is required at the time of service.
A one-time new patient consultation fee of $750 applies to all new patients and includes a comprehensive, one-hour visit.
An additional $750 fee applies for an in-depth review of your medical history and any past lab results, which occurs during your consultation to ensure the most thorough and personalized approach to your care.
Additional services including advanced diagnostic testing and specialized therapies—are not included in the consultation fee and will incur separate billing.
Agreement to Fees
As part of the booking process, new patients will be asked to review and agree to our fees in advance. This creates transparency and allows us to focus on individuals who are aligned and ready to move forward with care.
